• Church Expression • Key Goals for the next 12 months (Be specific, list your largest goals and due dates) • Length of Charter Requested: (typically one year) • Field Coach Name, Email, and Cell • Proposed times to Meet with Field Coach (at least quarterly) • Scheduled time of Formal Evaluation (PWR, at least three times per year) • Signatures (team leader, Evangel Field Coach, and Evangel Dean) 2. Once you have the information above complete your Evangel Charter document.

Note: Again, should you decide to add a team member later, it is highly recommended that you take the person through a formal process of commitment to the team and your vision. Other teams have suffered when members are added without a thorough understanding of, and commitment to, the team’s vision and goals.
